Facility Use

If you would like to use our Facilities, you will need to fill out the Facility Agreement form .  To locate the form, click on this link:   Facility Use 2023 .   You will need to provide Liability Insurance (or you can purchase some through a third party company listed in the packet) along with the completed form.  If you have any questions, you may contact Christy Riddle at 928-643-7333 x 312 or criddle@fredonia.org.
